Section 4 - Payment Information
We require a completed Direct Debit instruction from you to allow us to collect money owed from the activation of gift cards. Please either download or print the Direct Debit instruction, complete, sign and either upload below, send to us via email or post. Our address can be located on our website.
You will need to provide the bank account details in a separate email to services@hta.org.uk if you wish for this to be paid into a different account please tick below.

The money owed from redemption of gift cards and vouchers is paid by BACS to the bank account details provided on the Direct Debit instruction. If you wish for this to be paid into a different bank account please provide the bank account details in a separate email to services@hta.org.uk
